| Name: United Airlines Headquarters Location: Chicago, IL Designer / Architect: OWPP Description: United Airlines moved from its headquarters in Elk Grove, Illinois to the Willis Tower in Chicago. United takes up more than 10 floors in the Willis Tower, employing about 3,000 people. Design highlights include 200ft wide airplane wings in the 11th floor carpet.

| Name: Gene & Georgetti Location: Chicago, IL Designer / Architect: MDFI Description: Gene & Georgetti is a Chicago Steakhouse founded in 1941 by two men. Gene & Georgetti is located in River North and is Chicago’s oldest and one of its finest steakhouses. Mr. David’s Flooring has done much of its dining room carpeting to give Gene & Georgetti its elegant look.

| Name: Hillside Veteran's Home Location: Hillside, IL Description: Mr. David’s, in conjunction with the PFIA, donated flooring materials and supplies to help build a home for a disabled veteran. The tremendous efforts of all involved resulted in the completion of the hillside disabled veterans home project in only 86 days from the commencement of construction. On Wednesday, June 29, 2011, army SPC Sergio Lopez & his family were presented the keys to a brand new house in Hillside.

| Name: Central Dupage Hospital-Pediatric Short Stay Suite Location: DuPage, IL Designer / Architect: RTKL Contractor: Leopardo Description: The Pediatric Short Stay Suite at Central DuPage Hospital was designed for kids of all ages. The design concept of large, over-scale circles and bold uses of color repeated in the floor, ceiling and walls creates interest, amusement, and diversion for young patients during brief stays at the hospital. The circle shapes in the floor mirror the soffits in various areas and provide a clean reflection to the fiber-optic star lighting. The flooring proves to be an integral aspect of the design with overlapping and intersecting circles creating beauty, movement, and a positive distraction throughout the corridors of the suite.
This project won top honors in the Healthcare category as well as Best Overall Project at the Starnet 2010 Design Awards.

| Name: Mannington Showroom Location: Chicago, IL Description: The showroom presents Mannington Commercial carpet and hard surface products in ways that appeal to thousands of interior designers at NeoCon. To be successful, the project needed to showcase products for specification, inspire, and stand up to very heavy show traffic. The colors and design underscore Mannington’s commitment to design inspiration, featuring the large-scale graphics, modern spirit and flexible sophistication of Sylvan Collection and the lush organics of Monarch. Hard surfaces included Earthly Elements hardwood in unique planks and squares, and a conceptual installation of BioSpec, a high recycled content, antimicrobial homogeneous sheet that demonstrates industrial products can be beautiful.

| Name: Aramark Location: Downers Grove, IL Description: The regional office of ARAMARK undertook a project of replacing approximately 2,400SY of carpeting and vinyl plank flooring. This was a challenging project as it was meant to brighten the existing area, as well as give the area a home-like feeling. It was further complicated because the curved and rounded pre-existing offices were to remain exactly as they were, with glass entrance doors that had very little clearance between the bottom of the door and the floor. The vinyl plank was expertly installed to solve the clearance problem under the door, and the end result was an inviting residential look.

| Name: Willard Hall Café - Northwestern University Location: Evanston, IL Description: University students requested an alternative late night food service venue that would operate along-side the existing cafeteria. A traditional dining hall during the day needed to morph into a late night café after dark. Different dining venues were highlighted through changes in flooring; supported by changes in lighting. The major circulation path is carved with slate tile. Dining vignettes are set off with carpet where installation methods change in each vignette to subtly enhance its uniqueness. A casual lounge is defined with the use of a wood boarder and a new stone fireplace; the coffee bar is defined with pebble tiles. High moisture content in the slab was creatively overcome to allow the use of slate, luxury vinyl, porcelain tile and carpet. The design solution is respectful of the historic fabric and design philosophies of the surrounding buildings.

| Name: O’Carroll & Associates Location: Glen Ellyn, IL Description: All design elements were chosen to enhance the Frank Lloyd Wright style of the new Podiatry Surgical Center building. The carpet was selected before the other flooring materials in a neutral tone to complement dark wood tones. Low maintenance vinyl products were key in the design, as well as their ability to hide dirt in higher traffic areas. The porcelain tile, resembling slate, was similarly chosen for its design versatility with the other products and its added style. The luxurious combination of textures and colors create an overall sophisticated, rich-looking interior for the space.

| Name: West Side Christian Church Location: Springfield, IL Description: The design intent for the children’s wing of West Side Christian Church was to provide an exciting place where kids and parents would want to come and hang out, while encompassing the DNA of the church and their surrounding community. The use of the luxury vinyl wood plank flooring gives the space a warm yet clean feeling that is pleasing to all ages, while providing easy maintenance. The use of the floor transitions and the patterned carpet adds interest, excitement and easy way-finding for the families.

| Name: Trinity University Location: Deerfield, IL Description: Trinity hall was one of the newest dorms built about ten years ago. The carpet had taken a toll from the active dorms lifestyles. Trinity Hall was built with an interesting contemporary design of round porthole windows in the entrance doors on each floor, giving it a nautical influence. The halls are wide with entrances to the dorm rooms positioned recessed and across from each other. The passage through the hall creates a rhythm of space interrupted by the entrances to the rooms. It was decided that flooring much more durable than carpet was needed to sustain the foot traffic. We selected products from Centiva that resembled a grayed oak boardwalk interrupted by a fun bright blue water textured tile at the entrances into the rooms. This change provided a relationship to the nautical windows and a fun, yet practical flooring solution.

| Name: Bethany Methodist Hospital of Chicago Location: Chicago, IL Description: Methodist Hospital sought a transformation of its aging facility and image. The hospital wanted to create an upscale patient-focused care department. They also wanted the project to incorporate the latest designs to reduce falls, increase infection control, and enhance patient satisfaction. The project consisted of transforming existing semi-private patient rooms into six private patient rooms with large bathrooms and integral showers, a family room, nurse’s station, consultation room and various support spaces which are code compliant. The 4,000 square foot project was completed in a fully functioning hospital with the ER on the floor below.

| Name: Kraff Eye Institute Location: Chicago, IL Designer / Architect: Mr. Davids Description: This project consisted of replacing dated carpet in the public spaces of a large ophthalmology practice.
One of the challenges was finding a patterned carpet tile that would blend harmoniously with the existing wall finishes, which consisted of about seven different schemes, furniture, upholstery fabrics, and remaining existing burgundy carpet. The client had also dismissed their space planner. Another challenge that came up was with coordinating the work schedule with the doctors, furniture movers, telephone, electrical, IT and medical equipment technicians.

| Name: Ortho Molecular Products Location: Woodstock, IL Description: When designing the space for Ortho Molecular Products there were several considerations that we took into account regarding the building shell. OMP is a manufacturer of vitamin supplements. The space that we designed for them is their headquarters. We needed to create a high quality corporate office environment within a strict budget in an existing high bay warehouse facility. We took advantage of the high bay shell to create interesting interior volumes and spaces. We used rich colors and finishes to give it an upscale corporate feel, and used a combination of porcelain tile and carpeting to delineate the different areas within the building.

| Name: Good Shepherd Church Location: Naperville, IL Description: The vision statement for Good Shepherd Lutheran Church is ‘Reach and transform the spiritually distracted families in our local community so they come to know and love God.’ In order to help Good Shepherd obtain this, the community needed to play a vital role in the flooring selection. Because Good Shepherd is in a mid –to upper-class neighborhood, luxury vinyl flooring with stone-look was selected for the main lobby area. The look of the floor fit with the community, while durability, maintenance and cost benefited the church. The combination of ultramodern carpet tile and broadloom gives the space a classy warm feeling.

| Name: Northern Suburban Special Education District Location: Highland Park, IL Description: The Northern Suburban Special Education District provides a continuum of highly structured, individualized therapeutic programs designed to meet the educational and emotional requirements of its 6th - 12th grade special needs students. The building is designed to provide learning activities to students which enhance their competencies to lead satisfying lives at home, school, and in the community. The original space was over 30 years old, dark with low ceilings with a disjointed layout. The space was opened up and brightened. Material and color selection for the school were based on the sensory nature of the students. The floors are a high performance carpet. Colored pattern was selected to enhance the beauty and to help the students adapt.

| Name: Access Community Health Network Location: Chicago, IL Description: Access Community Health Network’s recently relocated ACCESS Cabrini Family Health Center provides a full continuum of care to its community. The site has strong obstetrics and gynecology, and pediatrics practices. The patient flow is divided between the pediatric side and the adult acute care and Ob-Gyn care with lively colors, patterns and textures creating an inviting, stimulating atmosphere for patients of all ages and their families. The practices have separate waiting rooms, vitals and exam rooms with shared, complimentary color palettes. The design improves the patient and family experience and enhances the ability to provide great patient care by improving patient privacy, safety, waiting times, efficiency, staff satisfaction and overall well-being.

| Name: Elmhurst Christian Reformed Church Location: Elmhurst, IL Description: Elmhurst Christian Reformed Church's new worship space had to have a visual interest for both children and adults, as all of the spaces needed to be very flexible for all of the different programs the church offers to a variety of ages. Warm, inviting colors were used with accent wall colors in many rooms. Flooring was accentuated with borders and soft curves, even a "pathway" in one area. The overall space is enlightening and fosters Fellowship and Worship.

| Name: Chiaravalle Montessori School Location: Evanston, IL Description: The Chiaravalle Montessori School is located in a three-story, historic building. Through multiple phases, the interior is being transformed into a modern and welcoming space. Bright, crisp colors accented by white moldings give the rooms a light and airy feeling sprinkled with whimsy for the children, such as vine room separators and colorful inlays in the floor. The design also features environmentally sustainable flooring and other “green” elements.

| Name: Renaissance Cleveland Location: Cleveland, OH Description: This sophisticated downtown Cleveland hotel is located on Public Square and is connected to Tower City Center's exclusive boutiques, restaurants and cinema. Within walking distance to Progressive Field, Quicken Loans Arena and the Rock & Roll Hall of Fame, the Renaissance's luxury hotel in Cleveland, Ohio features vaulted ceilings, high arched windows and a beautiful marble fountain. A city landmark, the historic Renaissance Cleveland Hotel was built in 1918 and its architecture reflects Cleveland's unique style and rich history.

Showroom Location: Itasca, IL Designer / Architect: Mr. Davids Description: A flexible space showcasing our flooring installation capabilities, the design focused on this and our owners, who are sustainability advocates, innovators and art collectors.
It was designed as if it is an art gallery with complex flooring installations as works of art. To have the space embrace tradition and innovation, modern materials & classic millwork details were used.
The low ceiling challenged the designers to provide flexible yet functional lighting. Their solution was to expose & paint the ceiling a bright white and use a combination of indirect/direct fixtures and accent fixtures.

Description: Located in the Merchandise Mart in downtown Chicago, this flagship Atlas Showroom showcases not only Atlas' exquisite carpets, but also Mr. David’s ability to complete perfect installations in unique conditions. Carpet that flows from the floor to the ceiling, and from the wall to the tops of tables protruding from the wall to create a unique work surface, are some of this installation’s unique features.

| Name: St. Alexius Hospital Waiting Rooms Location: Chicago, IL Description: The waiting rooms at St. Alexius Hospital are an intentionally stark contrast to the rest of the hospital, geared toward the comfort of patients and loved ones. While the facility transitions from all carpet to all resilient, the waiting rooms were refinished with padded carpeting. The colors in the carpet match various accents in the resilient throughout the hospital, but the waiting rooms have a much stronger design esthetic with the brightly patterned tiles. The goal was to create a warm and inviting space with a bold design to stand out from the rest of the hospital.

| Name: St. Alexius Mental Health Center Location: Chicago, IL Description: The Alexian Mental Health Center’s recent update is the epitome of a unique installation challenge. A long lease left the interior dilapidated and unattractive. The landlord provided an extremely limited budget for the modernization, which gave no allowance for overtime, thereby forcing the carpet installation to take place during normal business hours. Starnet carpet tile allowed a smooth transition from the old broadloom during daytime installation as well as created an attractive, affordable design. Alexian appreciated the stylish change from traditionally low-end, boring carpet to a beautiful, high-quality option at an equally reasonable price.

All have one thing in common – expertly installed plush patterned carpet installed by Mr. David’s on schedule and with minimal downtime to the hotel.

| Name: Gift of Hope Location: Itasca, IL Designer / Architect: Designer Office Concepts Contractor: Leopardo Description: Gift of Hope changes lives with their work and offers hope in time of difficulty. Not only was it important to provide a warm feeling for members of the general public, but also to instill that same feeling throughout the organization. Using vinyl wood plank flooring, the designer brought in warmth and banished the institutional feeling while providing way-finding for employees and guests. Strategic breaks in colors, patterns and wood vinyl allowed the designer to segment long expansive hallways, while adding dimension and interest to the space.

| Name: St. Alexius Hospital Location: Chicago, IL Designer / Architect: Proteous Group Description: The cosmetic renovation of two existing Labor and Delivery hospital units had to be completed without shutting it down. Only two rooms could be put out of commission at any one time and pathways to and from delivery rooms and operating room transfer had to be maintained at all times. Using plank flooring material made it easier for the contractor to stage corridor flooring replacement while maintaining the 24 hour work flow of these units. The contractor working closely with the staff successfully coordinated room shut downs and alternate corridor movements to maintain their operational modes.
